Consent Agreement for the Transmission of Collected Data to Partner Companies
This Consent Agreement (« the Agreement ») is entered into between ACHIEVEMENT INDUSTRY (the « Provider ») and ... (the « Data Subject ») to govern the transmission of data collected by the Provider to partner companies (the « Clients ») in accordance with the provisions of the General Data Protection Regulation (GDPR).
1. Free and Informed Consent
By signing this Agreement, the Data Subject confirms having been fully informed and understanding the specific purposes for which their personal data (« Data ») will be shared with the Clients. The Data Subject gives their free, informed, and specific consent for the transmission of their Data to the Clients in accordance with the terms and conditions set forth in this Agreement.
2. Purposes of Data Transmission
The Data will be transmitted to the Clients for the purpose of enabling the achievement of specific purposes mentioned in separate agreements concluded between the Provider and each Client. Purposes may include, but are not limited to, market analysis, customization of offers, provision of specific products or services, and any other purpose agreed upon between the Provider and the Clients.
3. Scope of Data Transmission
The Provider will only transmit the Data necessary for the achievement of purposes agreed upon with the Clients. Data may include information such as name, address, email address, phone number, preferences, and any other information necessary for the achievement of agreed-upon purposes.
4. Data Retention Period
The Provider undertakes to transmit the Data to the Clients for the duration necessary for the achievement of agreed-upon purposes, in accordance with legal provisions and agreements concluded with the Clients. Once the Data is no longer needed for the agreed-upon purposes, the Provider undertakes to delete or anonymize the Data.
5. Rights of the Data Subject
The Data Subject is informed of their rights under the GDPR, including the right to access their Data, rectify them, erase them, limit their processing, and transfer them to another data controller. The Data Subject can exercise these rights by submitting a written request to the Provider in accordance with the contact details provided in the Privacy Policy.
6. Data Security
The Provider implements appropriate technical and organizational measures to ensure the security and confidentiality of the Data transmitted to the Clients. The Provider also ensures that the Clients adhere to similar security measures to protect the Data against unauthorized access, disclosure, alteration, or destruction.
7. Revocation of Consent
The Data Subject hereby revokes their previously given consent for the transmission of their Data to the Clients. This revocation applies to all purposes for which consent was initially granted, as specified in the previous consent agreement.
The revocation of consent takes effect from the date of receipt of this Revocation by the Provider. From this date, the Provider is no longer authorized to transmit the Data Subject's personal data to the Clients, except as provided by law or on the basis of another legal basis under the GDPR.
The Provider will confirm in writing the receipt of this Revocation to the Data Subject as soon as possible. This confirmation will be sent to the email or postal address provided by the Data Subject, unless otherwise indicated.
